What Happens Normally After the Pill

After taking the standard medication regimen (mifepristone followed by misoprostol), it is typical to experience uterine cramping, vaginal bleeding, and the passing of blood or clots. Bleeding and cramping are often strongest a few hours after the second medication, then gradually lessen over the following days or weeks.

Mild, short-term side effects, such as low-grade fever, mild nausea, diarrhea, or light dizziness, can also occur and may resolve without medical treatment.

When Bleeding or Clotting Is a Concern

Seek emergency care if you experience:

  • Vaginal bleeding heavy enough to soak two or more full-size pads per hour for two hours
  • Passage of large clots (for example, lemon-sized or larger), especially if ongoing
  • Heavy bleeding that does not decrease over time or suddenly becomes worse

These signs may indicate excessive bleeding or retained pregnancy tissue, both of which require prompt medical evaluation.

Pain, Fever, or Signs of Infection

While cramping is expected, certain symptoms point to complications. Seek immediate care if you experience:

  • Severe or worsening abdominal or pelvic pain that does not improve with recommended pain relief
  • A fever of 100.4°F (38°C) or higher lasting longer than 24 hours
  • Chills, body aches, unusual weakness, or feeling generally unwell
  • Foul-smelling or unusual vaginal discharge, especially when combined with pain or fever

Additional Warning Signs

Symptoms such as dizziness, fainting, extreme weakness, or a very rapid or irregular heartbeat may indicate significant blood loss. These require immediate evaluation in a hospital emergency department.
